Sports Physicals

Sports Physicals and Health History Updates for students participating in winter sports are due on February 18.
In the event that you are not able to schedule a physical with your private physician, our school doctor will be available to perform spring season sports physicals in the Health Office. Pre-registration is required.  Contact Mrs. Nill ( ext 114 or lnill@rumsonfairhaven.org ) Part A of the physical form must be completed by parent in advance of the physical.

VERY IMPORTANT – PLEASE REVIEW ENTIRE LETTER

 

Dear Parent(s)/Guardian(s):

Pursuant to and in accordance with N.J.A.C. 6A:16-2.2, all students must have a physical examination form reviewed by the school physician PRIOR to athletic participation (this includes “try out” practices).  New Jersey Administrative Code indicates the examination shall be conducted within 365 days prior to the first practice session.  No student will be permitted to practice/participate without a physical reviewed by our school physician.  All fully completed physical forms are due to Ms. Scisco in the Main Office according to the schedule below:

Students submitting physical forms to the Main Office after said deadlines WILL NOT be eligible to participate during the active season.  Coaches are not permitted to accept the completed physicals forms. 

For your convenience, we have attached the Athletic Pre-participation Physical Examination forms and a corresponding instructions sheet; these documents are also available online at http://www.rumsonfairhaven.org/athletics/athletic-physicals/.  

Additionally, any student whose medical examination was completed more than 60 days prior to the first practice session must provide a health history update.  This 60 Day Health Update Form is attached and is also available at our school’s website.

To ensure compliance with the State of New Jersey and the NJSIAA, the above procedures and deadlines must be adhered to. There will be no exceptions.
