Transition Coordinator:
Mrs. Stefani’s goal as our new transition coordinator is to work in conjunction with Case Managers, as part of the Child Study Team, as well as with the BCBA, teachers and related service providers to coordinate Transition needs for students with disabilities to achieve personal fulfillment. To provide students with guidance, counseling services, adult service providers, post secondary education/training opportunities, and community support to create each student's own personal, educational, and occupational transition plan. Mrs. Stefani will work with families in an effort to link their child to post-secondary support coordinators and/or community agencies.
